MPOWR case management serves as the “central nervous system” for cradle to career collaborative organization

NAZ exists to permanently close the achievement gap and end generational poverty in North Minneapolis. NAZ collaborates with organizations in the community and supports hundreds of low-income families as they put their children on a path from cradle to career.

Background
Minnesota has one of the worst achievement gaps in the nation and many of the negative indicators which include high rates of violence, poverty, unemployment, and failing schools are concentrated on the Northside of Minneapolis.

With a goal to end multigenerational poverty and provide comprehensive support from prenatal care to college and career, NAZ needed a case management system that could be used collaboratively among organizations to create holistic, actionable achievement plans.

Solution
NAZ has leveraged the MPOWR solution to facilitate collaboration among partners like early childhood centers, schools, healthcare facilities and non-profit organizations. According to the Wilder Foundation, for every $1.00 that has been invested, NAZ and its partners have succeeded in helping avert $6.12 in future social costs.

The North Minneapolis region was recently federally designated as a Promise Zone. North Minneapolis is one of eight federal Promise Zones in the country. The region received official designation during Secretary of Education Arne Duncan’s visit to Minneapolis in April 2015.

“Using the MPOWR technology solution is the only way we could accomplish true collaborative action,” said Michelle Martin, COO of NAZ. “We use the system to coordinate comprehensive support of a child or family through a team approach across schools, afterschool, health/behavioral health and whole family support programs. We track information provided through the system and effectively use that data to drive results in the solutions we offer.”

The designation as a federal Promise Zone gives North Minneapolis and NAZ a competitive advantage when applying for federal grants, so they can continue to do impactful cradle to career work in an area of the country that truly needs it. MPOWR continues to serve as the central software solution used to carry out NAZ’s mission.
